Eligibility Criteria
Applicants must fulfill the following conditions to qualify for the scheme.

• The family must belong to the Antyodaya category or economically weaker section.
• Annual family income should be below ₹1.80 lakh for free enrollment.
• Families earning between ₹1.80 lakh and ₹3 lakh can register by paying ₹1,500 annually.

• Aadhaar Card.
• Family Identity Card (PPP ID).
• Income Certificate.

Registration Process

• Carry Aadhaar Card, PPP ID, and income certificate.
• The CSC operator will fill out the Chirayu application form.
• After successful registration, the Chirayu card can be printed after a few days.
Registration Through Ayushman Helpdesk
• Visit a government or empanelled private hospital.
• Contact the Ayushman Mitra available at the helpdesk.
• Submit the required documents for verification.
• Once approved, the applicant can download the Chirayu card online.
Registration Through CSC VLE
• Residents of rural areas can apply through CSC Village Level Entrepreneurs.
• The operator will complete the registration process online.
Registration Through Antyodaya or Saral Center
• Visit the nearest Antyodaya or Saral Kendra.
• Submit required documents.
• The center operator will complete the registration.
Chirayu Card Download Process
After successful registration.
